Submit an e-Prescription

1. Search EMR system for Cascade Specialty Pharmacy and select the pharmacy located in Poulsbo, WA
2. Enter patient’s name, date of birth, and preferred contact number
3. Enter medication as desired (Okay to select powder form or manufactured alternatives.)
4. Utilize note section for medications not listed in your EMR section (Example: Okay to compound ________ at _______MG.)  Or for further pharmacy instructions call 360-229-7913.

We’ll take care of everything else!
